If you need the police report about an accident you were involved in to locate other parties or file an insurance claim, go to the police precinct where the accident occurred within 30 days to fill out a "Request for Copy of Aided/Accident Record" form. It costs $10 (check or money order). If 30 days have passed, write the DMV for the accident report
