Report Dirty and Unhealthy Public Places 
If you see unclean or unhealthy conditions in a restaurant, health club, pet shop, swimming pool or other public facility, report it.
Is your home or a nearby lot infested with rats or ticks? Have
you recently been grossed out by unclean or unhealthy conditions
in a restaurant, health club, pet shop, swimming pool or other
public facility? Are people smoking where they shouldn't be --
at the next table, in the cubicle next to you, etc.? Report it
to the Department of Health's Bureau of Central Complaints and
Information. Give the name and location of the facility and the
date and time you were there. You can also click
